New Patient Appointment Request

To request your first appointment online, please click the button below to request. You will be directed to another site to securely request an appointment. You may also call +1 469-513-5500 to schedule. You do not need a doctor’s referral to be seen at Texas Center for Proton Therapy.

If another doctor has referred you, one of our referral staff will call you to schedule an appointment at a convenient time for you. Many patients worry they won’t be able to see their primary care physician during treatment. We actually recommend you continue to see your primary care doctor.

If you are requesting an appointment for yourself or a loved one without a doctor’s referral, our patient intake team will need a copy of the patient’s medical record. Texas Center for Proton Therapy will gladly request records from your doctor for you. Download the medical records release form. Fax to 469-420-9619‍.
