Preparing For Your First Appointment

At your first appointment, you may need to complete several forms. To save time at your first visit, you may complete these forms in advance and bring them with you.

Please follow the steps below to complete the forms in advance:

Download the Forms: To access the forms, click on the provided links below. The forms are in PDF format, which can be easily opened and printed from most devices.
Complete the Forms: Carefully read each form and fill in all the required information. Ensure that all information is accurate and up to date.
Signature and Date: Some forms may require your signature and date. Please print then sign and date these forms where indicated.
Bring the Forms to Your Appointment: Once you have completed the forms, please bring them with you to your scheduled appointment.

New Patient Packets

The new patient registration packets are organized by specialty. If the specialty is not listed, please use the standard Texas Oncology New Patient Registration Packet. To request a packet in Vietnamese please notify your new patient coordinator.

Texas Oncology is currently implementing electronic registration (eRegistration) in many of our practices. If your appointment is at one of the locations that is already live you will receive access to new patient registration forms via email or text message automatically within 24 hours of your appointment being scheduled.
