Care Coordination and Follow-Up Care

Partnerships with physicians in the communities we serve are a hallmark of our practice. By referring your patient to our practice for their oncology or hematology needs, you will not lose a patient – we recommend your patients continue to see you for routine medical care.

We’ll keep you informed of your patient’s progress and treatment plans. When they’ve completed their treatment, we will provide you copies of their treatment records so you have a complete picture of their oncology care.

While we will continue to monitor your patient long-term, we recommend they return to you following treatment for their ongoing care.
